A Custom Knee Brace - Do You Really Need One?

A custom made knee brace may seem to be more effective since it involves numerous trips to see a brace specialist (called an orthotist). An orthotist makes a custom mold of the knee (including the upper and lower leg) and takes numerous measurements. Obviously, there are always exceptions to these conclusions. You always have to consider those cases where someone has a deformity, and an off the shelf knee brace just can't fit their leg. Moreover some of the exceptions may be, but are not limited to, anyone with a traumatic injury to their leg that causes deformity of the anatomy. If a person has ulcerations, it is possible that a custom made knee brace will provide a unique fit over that of an off the shelf brace. Knee Brace Composition - What tests are involved in diagnosing an ACL tear?

1.) The Lachman Test

An examiner who is skilled can often times detect an injury in your knee before ordering other tests like an MRI or an x-ray. One such manual test is the Lachman test. Named after John Lachman, M.D., the Lachman test is performed to test a person's ACL for stability. More specifically, the goal of this test is to help determine if the ACL has been torn or if it is still intact.

To perform the Lachman test, the patient should be laying supine (on their back) with their knee in approximately 20-30 degrees of flexion. The leg that is being tested should also be externally rotated. The individual conducting the maneuver needs to place one hand on the patient's thigh, while the other hand needs to be placed behind the tibia, with one thumb placed on the tibial tuberosity. The examiner pulls anteriorly on the tibia to determine forward translation of the tibia on the femur. (The tibia is the larger of the two bones in the leg, while the femur is the thigh bone.) When pulling anteriorly on the tibia, an intact ACL should prevent forward translation movement of the tibia on the femur. This is known as a "firm endpoint". On the other hand, if the end point is "soft", and the tibia can move forward on the femur, then this suggests that the ACL is torn.

2. ) The Anterior Drawer Test

A second commonly used test to determine the stability of the ACL is the Anterior Drawer Test. In this test, the patientis lying supine (on their back) with their hips flexed to 45 degrees and their knee to 90 degrees. The individual performing the test then grasps the tibia just below the knee, with their thumbs on either side of the patellar tendon. It is importantto note that the hamstring muscle group must remain relaxed to help ensure a proper test. (The index fingers can be used, by the examiner, to test the hamstrings). Once the examiner is in position, the tibia is then drawn forward anteriorly. When the examiner notes that one side has increased amount of anterior tibial translation compared to the opposite limb, this will help indicate whether an ACL injury has occured.

If these tests are positive and the ACL is sprained, or can be torn. Depending on the severity of the injury, a knee brace can help provide meaningful stability, and can help reduce pain as well. Osgood-Schlatters Disease & Methods of Treatment

What is Osgood-Schlatters Disease?

The Osgood-Schlatter Disease is claimed by some to not actually be a disease, but is rather a collection of symptoms that involves the tibial tubercle epiphysis. (The term epiphysis is essentially a growth center for bone). Discovered in 1903 by Dr. Robert Bayley Osgood and Dr. Carl Schlatter, Osgood-Schlatters Disease is often found among young athletes and children, mainly affecting boys and girls between 10-16 years of age. Osgood-Schlatters Disease can be characterized by inflammation, swelling, and/or painful symptoms at the tibial turberosity. Often times Osgood-Schlatters disease will subside when the individual stops growing, and the tendon becomes stronger. Moreover, the symptoms will be present for younger athletes and children during or after exercise.

What is the cause of Osgood-Schlatters disease?

Osgood-Schlatters disease occurs when the patellar tendon, (which attaches the quadriceps muscle at the front of the thigh and inserts on the tibial tuberosity) is overused. The tibial tubercle is a small bump on the tibia (your shin bone) just below your knee. Inflammation can occur when the patellar tendon tugs on the tibial tuberosity. Osgood-Schlatters disease is also known as a tibial tubercle apophyseal traction injury. Children and young athletes involved in activities that include a high frequency of running and jumping, are more at risk, because these activities put a greater strain on the patellar tendon.

Tests & Diagnosis

Upon visiting with the physician, your child will have a physical examination of their knee. The physician will be looking for symptoms such as redness, pain, tenderness, or swelling in and around the knee joint. He or she will also want to check the range of motion (ROM) in your child's knee. Further tests, such as x-rays may be used to look at the bones that comprise the knee joint and leg, in an effort to more closely examine the point where the patellar tendon inserts on the tibia tuberosity.

What can the athlete do if they have Osgood-Schlatters disease?

Seek the advice of a sports injury professional. Before proceeding with any treatment, it is important to make sure that Osgood Schlatters is causing the pain. Your physician can confirm or disconfirm this diagnosis. Treatment is usually conservative; rest and simple pain reduction measures of ice packs can be of great assistance. What Is "Water On The Knee"?

The term "water on the knee" is a generic term that describes the accumulation of excess fluid (edema) in or around the knee joint. As a result of a trauma, overuse, or other underlying conditions or disease, water on the knee may exist.

Often times, when a person has water on the knee, the underlying condition is arthritis. The term arthritis means "joint inflammation". People also refer to arthritis as being "joint pain" as well. The two most common types of arthritis are osteoarthritis (OA) which effects over 27 million people a year (mostly women), while rheumatoid arthritis (RA) effects over two million adults. Most of them being women over the age of 45. Although these two types of arthritis effect millions of people, and are the two most common types of arthritis, there are over 100 different types of arthritis.
